Software startup Healthify announced the completion of $6.5 million in Series A financing. Three investors participated in the round: BlueCross BlueShield Venture Partners, Primary Venture Partners, and Active Venture Partners.

The Healthify platform helps managed care plans and Medicaid case managers address social needs among their members to reduce costs, boost quality metrics, and improve case management. The platform offers a dynamic screening tool to assess a patient’s social needs and identify who is most at risk. It uses this data to refer patients to the best resources for their needs and automates the follow-up process via interactive texts. This information is then relayed back to case managers via a dashboard that makes case management easier.

According to the company, Healthify will use funds from the financing to expand its care coordination product suite and expand its network of customers and social service partners.
